Integrating Diabetic Retinopathy Management with Diabetes Mellitus Care in Baringo County, Kenya

Description

Background: Diabetes Mellitus (DM) incidence is increasing in Kenya and can lead to complications such as diabetic retinopathy (DR). DR is currently the leading cause of vision loss in working-age adults worldwide. The risk of developing DR can be reduced by early eye screening, blood sugar and blood pressure control. A 2018 survey in Baringo reported DR prevalence of 24.5%. Baringo County implemented an integrated approach to management of DM/DR. The aim was to increase uptake of DR services among DM patients through early screening and management, health education and community engagement.

Methods: The interventions were carried out at the Baringo County Referral Hospital and select sub county hospitals from 2019-2022. The interventions included screening, referral & follow up of DR cases; capacity building of eye care staff on DR grading, fundoscopy, laser treatment and intra-vitreal treatment. Equipment for screening and treatment of DR were procured and patients support groups formed. Stakeholder engagement forums were also conducted to sensitize on DM/DR services.

Results: Among the 2,302 diabetic patients screened, 666 (28.9%) had DR with 833/1463 (57%) being female. Among the DR patients 299/505 (57%) were female. A total of 298 and 331 DR patients were treated with intra-vitreal injection of anti-VEGF and laser respectively. Regarding training of eye care staff, 7 staff were trained during the period. Six community awareness sessions and screening services were conducted, 6 patient support groups were formed. These support groups facilitated income generating activities such as table banking, poultry keeping, peer education, counselling, home visitation and adherence support. Procurement of select eye care equipment was also done and training of 81 peer educators. Stakeholders engaged included the Kenya defeat diabetes association, local political actors, religious leaders and various interest groups
