BACKGROUND
Universal health coverage is essentially health care made universally accessible to individuals and acceptable to them, through their full participation and at a cost they can afford. Primary care network is one of the models to achieve this.
Suna west is a sub county in Migori county with a total population of 163,077 ,4 wards with 2 sub county hospitals (Hub and mini hub) and 13 dispensaries (spokes) and an acute shortage of staff especially specialists. The MDT identified this as a gap and organized for in reaches in the spokes to screen, review and booked clients for referral with complicated conditions to the hub for review by specialist sourced from the referral hospital and surrounding private facilities.
Objectives
•To sensitize community social health insurance.
•To screen, review and make appropriate referrals for patients with complicated diseases conditions.
Methodology
This is a descriptive study design of the MDT in reaches in the spokes and expert in reaches at the hub and mini hub.
Currently 11 MDT and 2 Expert in reaches in the hub and mini hub have been conducted across all the four wards.
Results
Currently 11 MDT in reaches have been done in which 937 patients were reviewed, 171 referred for specialized services. A total of 462 clients were registered on SHA.
2 Expert in reaches conducted of which 264 patients were reviewed, 50 clients referred for specialized procedure and 83 enrolled on SHA.
The facilities in which in reaches were done experienced an increase in work load afterwards due to marketing of their services under SHA.
Conclusion
Primary care network approach in PHC is a a game changer in increasing access to specialized care in low resource set up.
