Impact of Carbohydrate Counting Education on Glycemic Control Among Children and Adolescents with Type 1 Diabetes in Kenya: Evidence from DMI Centre

Background
Carbohydrate counting supports improved glycemic control in children with Type 1 Diabetes Mellitus (T1D). However, its uptake and impact in low-resource settings like Kenya is limited.
Objective
To assess changes in glycemic control following carbohydrate counting education among children and adolescents with T1D at DMI Centre.
Methods
A prospective observational review was conducted among 54 children with T1DM receiving care at the DMI Centre. Baseline HbA1c values were recorded in December 2024, followed by structured carbohydrate counting education sessions led by licensed dietitians. Patients and caregivers were taken through individualized and group-based practical lessons on portion estimation and insulin adjustment based on Insulin to carb ratio. Follow-up HbA1c measurements were obtained in March 2025. Glycemic control was categorized into seven standard HbA1c ranges. Descriptive analysis was conducted to compare distribution shifts between baseline and post-intervention.
Results
The proportion of patients in the 10.1–12.5% range declined from 41% to 28%. Those in the 7.1–10.0% range increased from 24% to 43%. The proportion achieving ≤7% decreased slightly (11% to 9%), while those in the >14% category dropped from 7% to 6%. Moderate improvements were also noted in the 13.1–14% range (11% to 7%). The daily activity reporting tool had a 70% adoption rate, and the enrolment tool reached 67%.

Conclusion
Carbohydrate counting education led to improved glycemic control among T1DM patients, with fewer in poorly controlled categories. Though optimal control rates remained stable, the shift toward moderate ranges and lower extremes signals early program success.
