Background
Cardiovascular diseases (CVDs) are the leading cause of death globally, taking an estimated 17.9 million lives each year. More than four out of five CVD deaths are due to heart attacks and strokes, and one third of these deaths occur prematurely in people under 70 years of age
There is a shifting trend of non-communicable disease and the CVD is a major public health challenge in Kenya, including Homabay. We assessed the pattern of cardiovascular disease admissions and outcome among patients admitted in 2024 at Homabay County Teaching and Referral Hospital.
Methodology
We retrospectively abstracted records of clients with cardiovascular disease admitted in the medical wards at HCTRH in 2024. Sociodemographic and clinical variables were abstracted from Ministry of Health registers into a database with enforced data quality and consistency checks. Sociodemographic of interest were sex, age while clinical variables were CVDs admissions, comorbidity and mortality. We conducted descriptive data analysis assessed pattern of cardiovascular disease admissions and mortality in the medical wards
Results
Out of 3645 annual medical admissions in 2024, 12% (437) involved cardiovascular diseases (CVD). The mean age of patients was 67 years (SD ± 18.7),females accounting 66% of cases. The average stay in the ward was 6 days (SD ± 5.1). Among CVD cases, heart failure accounted for 47.1%,cerebrovascular accident (CVA)12.4%, hypertension 6.6%, transient ischemic attack 4.6%, hypertensive heart disease 2.5%, ischemic heart disease 2.1%, venous thromboembolism 2.1%, conduction disorders 1.4%,rheumatic heart disease 1.1%.HIV coinfection was present in 19.3% of these cases. The mortality rate was 15% (67 cases), with 52% of deaths caused by heart failure.
Conclusion
CVD are a major significant cause of morbidity and mortality at HBCTRH, with heart failure leading in admission and mortality. Early detection, improved outpatient care and disease control,secondary prevention programs in patient’s management may help reduce admissions
